• What We Believe (Agreement to comply with Academy standards)
• Parent Statement of Cooperation
• Copy of South Carolina Certificate of Immunization
• Minor Release Form
• Complete Medical Release Form
• Pickup and Emergency List
• Final Interview
• Placement Testing May Be Required for students entering grades 1st – 3rd.
• Immunization Record
